INNOMINATE ROTATION & LOWER-LIMB CHAIN: A

BIOMECHANICAL STORY FROM PELVIS TO FOOT

Innominate rotation represents a key sagittal-plane and transverse-plane pelvic dysfunction that strongly influences whole-body biomechanics. Whether the pelvis is held in anterior or posterior innominate rotation, the effect is never local-it alters spinal alignment, hip muscle length-tension relationships, femoral mechanics, knee loading, and even foot posture. The pelvis acts as the central transmission hub between trunk and lower limb, so any asymmetry here propagates both upward and downward through the kinetic chain.

In an anterior innominate rotation, the involved hemipelvis rotates forward around the sacroiliac axis. Biomechanically, this increases functional hip flexion bias and often exaggerates lumbar lordosis on the same side. Muscles such as the iliopsoas, tensor fascia latae, and adductors tend to become short and overactive, pulling the pelvis forward and maintaining the rotated position. At the same time, posterior stabilizers like the gluteus maximus become lengthened and mechanically disadvantaged, reducing their ability to generate hip extension torque during gait.

A posterior innominate rotation, on the other hand, biases the hip toward relative extension and flattens the lumbar curve on that side. This shifts load transmission posteriorly through the sacroiliac joint and lumbar spine. The hamstrings and adductor magnus often become short and dominant, anchoring the pelvis backward, while anterior stabilizers lose optimal length-tension efficiency. This imbalance alters stride mechanics, often shortening step length and reducing elastic energy storage during walking or running.
Frontal-plane control is heavily influenced by the quadratus lumborum and gluteus medius, especially during single-limb stance. When one innominate rotates asymmetrically, the gluteus medius on the affected side may become long and weak, reducing its ability to control pelvic drop. Biomechanically, this causes compensatory trunk lean or excessive hip adduction and internal rotation during stance phase, increasing stress across the hip joint and lateral knee structures.

These pelvic changes strongly affect femoral and knee mechanics. Altered pelvic orientation changes acetabular positioning, which modifies the direction of femoral force vectors. This often manifests as increased femoral internal rotation, dynamic knee valgus, and altered tibial rotation during weight-bearing activities. Over time, these abnormal loading patterns increase strain on the medial knee, patellofemoral joint, and surrounding soft tissues.

At the distal end of the chain, foot biomechanics adapt to pelvic asymmetry. An anteriorly rotated innominate is commonly associated with increased pronation, as the body seeks ground contact stability by collapsing medially at the foot.

🦾Upper Limb Movements & Biomechanics - One Arm, Many Actions

This image is a great reminder that the upper limb is not just a collection of joints, but a highly coordinated biomechanical system. From the shoulder to the wrist, every movement you see here is governed by precise joint axes, muscle synergies, and neural control.

At the shoulder (glenohumeral joint), movements such as flexion, extension, abduction, adduction, and internal-external rotation occur around multiple planes. The shoulder sacrifices bony stability for mobility, which is why coordinated activity of the rotator cuff and scapular stabilizers is essential to keep the humeral head centered in the glenoid during motion. Any imbalance here increases shear forces and injury risk.

Moving to the elbow, flexion and extension primarily occur at the humeroulnar joint, functioning like a hinge. However, elbow mechanics cannot be isolated from the forearm. The position of the shoulder and forearm rotation directly influence elbow loading, especially during lifting, pushing, or throwing tasks.

The forearm introduces a unique biomechanical feature:

pronation and supination. These movements occur due to the radius rotating over the ulna, allowing the hand to be positioned optimally in space. Supination is crucial for forceful gripping and lifting, while pronation is dominant during reaching, pushing, and weight-bearing activities. Poor control here often transmits excessive stress proximally to the elbow and shoulder.

At the wrist and hand, flexion and extension fine-tune force transmission and precision. The hand does not simply follow the arm-it adapts dynamically to task demands. Efficient wrist positioning optimizes muscle length-tension relationships, improving grip strength and endurance.

From a kinetic chain perspective, dysfunction at any one segment-shoulder stiffness, weak rotator cuff, poor forearm control-creates compensations throughout the limb. This is why shoulder pain may originate from the wrist, or elbow pain from faulty shoulder mechanics.#Prosthetics

Scoliosis - A Clinical & Biomechanical Perspective for Coaches & Rehab Professionals

Scoliosis is a three-dimensional spinal deformity, not just a side curve. It involves:

Lateral deviation

Vertebral rotation

Altered sagittal alignment

(Clinically diagnosed when Cobb angle > 10°)

Q Main Types:

Idiopathic (most common - adolescent type)

Congenital (vertebral malformations)

Neuromuscular (e.g. CP, muscular dystrophy)

Degenerative (adult-onset)

*Why it matters functionally:

Asymmetric loading & muscle imbalance

Altered motor control & proprioception

Compensatory movement strategies

Possible pain, fatigue, and reduced efficiency

Key Insight:

Scoliosis is not only structuralbut also neuromuscular Strength training alone does not correct it-and poorly planned training may worsen symptoms.

Important Clarification:

Poor posture or daily habits do not cause structural scoliosis, but they can create functional/postural deviations that mimic it or aggravate existing curves.

Training Goal:

Not "straightening the spine," but optimizing function, movement quality, and long-term spinal health.

The patient had a history of qaudraplegia due a RTA

The result with fitment is unexpected but unfortunately due to patient confidential i didn't add fitment photo IN Assesment

Ankle - heel varus and forefoot adduction, and cuvas mid foot TA tightness L > R

KNEE - no muscle power, slightly spasticity hamstring and semitendinous & semimembranosus

unstable trunk control(spinal) and neck control(neck collar)

RX - HKAFO with bail lock (Becker) with solid ankle

3D printing has been around for a while now. The question is, has it truly become accessible across different markets, especially in Africa?

We've seen a similar pattern before. Silicone medical devices

came with promise, knowledge was not available, and professionals could not offer it, even though the training is possible, adoption remained slow. Not because the technology didn't work, but because access, training, and confidence created barriers.

With 3D printing, the knowledge is everywhere. Tutorials exist. Tools exist. Case studies exist. Yet professionals themselves often become the final bottleneck, not out of resistance, but out of uncertainty. New technology challenges familiar workflows, and that hesitation is understandable. But is 3D really new?

The encouraging part is what we're seeing now. More prosthetists, orthotists, and clinicians are beginning to try 3D printing for their patients. More are ready to experiment, learning, and adapting the technology to real clinical needs, and that's why we are focused on becoming the infrastructure of how technology can grow.

Progress may be gradual, but it's real. And each professional who takes that first step brings us closer to a future where advanced tools are not just available, but truly accessible and locally owned.
